Can hair transplant be done without donor area presence?

Firstly, we should understand that what is donor area? Donor area is an area which generally has permanent hair bearing and these hair grafts can be transplanted into recipient or the balding area to provide permanent hair in these areas of baldness.
The adequacy of the donor grafts will depend on the area of hair loss which means that the number required to cover the balding area can be extracted or not from the donor area of the scalp. In the scalp the donor area is the back and sides of the scalp. This is the most valuable area for the number and the quality of the donor grafts. But if this area is not adequate then grafts can be extracted from other areas and mixed with the scalp grafts which are then transplanted. Other areas from where the hair grafts can be extracted are beard, upper chest and upper back.
In total absence of scalp donor like in total baldness one can do hair transplant with extended donor areas but the results may not be that pleasing so it is better to avoid doing hair transplant in total absence of the main or primary donor area.
